The 2021 Clinical Practice Guidelines are nationally recognized and based on latest clinical evidence. They were reviewed by HPSJ’s Medical Director and have been reviewed and adopted through the QM/UM Committee of HPSJ’s governing Commission.
We utilize them to design our Disease Management and Care Management programs for
HPSJ members. The Clinical Practice Guidelines also are being provided to our network
physicians to use in taking care of our members.
About the 2021 Clinical Practice Guidelines –
- Clinical Practice Guidelines are about defined clinical best practice, such as desirable diagnostic tests, the optimal treatment regimen for a specific diagnosis, and links to resources.
- Additions include new conditions added to case management, several new health education program areas and more links to resources.
- They now are posted at www.hpsj.com/clinical-practice-guidelines.

attainable. To that end, HPSJ offers tools to assist our providers in meeting and maintaining proven best practices, beginning with Clinical Practice Guidelines.
