If you live in Alpine, El Dorado, San Joaquin, or Stanislaus counties and are eligible for Medi-Cal, you can become a Health Plan of San Joaquin/Mountain Valley Health Plan (“Health Plan”) member! At Health Plan, you will have access to high-quality health care in a timely, caring, and culturally sensitive manner.
