Health Plan of San Joaquin/Mountain Valley Health Plan (“Health Plan”) is excited to introduce a brand new program that rewards you for getting your yearly checkups and routine exams. We offer preventive care at no cost to you. Better yet, we reward our members for taking steps to be healthier. Our goal is to help you achieve your health goals and maintain a healthy lifestyle.

What is myRewards?

myRewards is an incentive program for selected Health Plan members who have not visited their doctor or stayed up-to-date with their health exams and check-ups. Members visit the doctor and text Health Plan proof of their visit. Health Plan checks member’s proof and sends an email to the member with a link to the reward gift card! Questions? Do I qualify for myRewards?

You may qualify for a reward if:

  • You are a member of Health Plan
  • It is your time for a checkup or test that HPSJ is part of the myRewards program. See a list below.
  • You have been told it is time for the checkup or test and complete the visit on time as required.
How do I get my gift card?

1. Go to Health Plan’s website to log into your myHPSJ Member Portal.
2. Make a member portal account.
3. Fill out the myReward form. You will be asked about what service you received, the date of service, and information about the provider who ordered/completed services.
4. Press “Submit”. You will have the option to have the card emailed, texted, or mailed to you.

myRewards FAQ

1. I want to know if I qualify for HPSJ’s myRewards. You may qualify for a reward if:

  • You are a member of Health Plan of San Joaquin
  • It is your time for a checkup or test that HPSJ has in the myRewards program.
  • You have been told it is time for the checkup or test and complete the visit on time as required.

2. I don’t know how to get my gift card. You can go to HPSJ’s website to log into your myHPSJ Member Portal. Fill out the Reward form. Press “Submit”. Next, you will receive a link to the gift card you selected.

3. I have questions about my reward gift card. View the status of your gift card through the myHPSJ Member Portal. You can also call our customer service team at 888.936.PLAN (7526)

4. What if I change my mind about the Reward/gift card I selected? Make sure the Reward you select is the one you want. HPSJ can’t change the Reward after you submit your request.

5. Will the link to my gift card or my gift card expire? Links to the reward gift card(s) will expire after 6 months of the date when you get the link. If you do not claim the link within the 6 months, the card is cancelled. Gift cards from Walmart do not expire. If you cannot open a link or have other issues, call HPSJ’s customer service at 888.936.PLAN (7526).

6. I was charged by my phone carrier for getting text messages from HPSJ. I want to get reimbursed. If you were charged by your carrier for text messages sent by us, please let us know and we would be happy to reimburse you. To request a refund please log into your myHPSJ Member Portal and fill out the Fee Request form.

7. Does my doctor have to sign off on my exam or test before I can get my gift card? To get your Reward, you will have to tell HPSJ when the checkup or test was completed and the name or location of where the visit was completed.  HPSJ will work with your doctor or clinic to confirm your visit as needed.

8. I am having issues getting my gift card. Check the status of your reward through the myHPSJ Member Portal. You can also call HPSJ’s Customer Service Department at 888.936.PLAN (7526).

9. What is my member ID number? Where can I find it? All HPSJ members have a 9 digit number that helps identify them in HPSJ’s system. The number starts with “200”. You will need to enter your HPSJ Member ID in the form when you request your rewards. If you don’t know your Member ID number or don’t know how to find it, we are here to help you. Qualifying Exams

If you would like to know if you qualify for other Health Plan rewards please take a look at the list of available exams that you and your family can benefit from. Get rewarded for being healthy!

Well Visit

Take your child to the doctor for his/her YEARLY exam and get a $25 gift card.

0-15 Month Olds:

Your baby must have six well child exams by the time they are 15 months old. Take your child in for a well child visit anytime this year and you can submit this rewards request.

15-30 Month Olds:

Your toddler is growing up! Your child needs to see the doctor twice between 15 and 30 months. After the 2nd visit you can submit this reward request.

By age 2 children should have the following vaccine series:

  • DTaP (Diphtheria, Tetanus and Acellular Pertussis)
  • IPV (polio)
  • Measles, Mumps, and Rubella
  • HiB (haemophilus influenza type B)
  • Hepatitis B
  • Chicken Pox
  • PCV (pneumococcal conjugate)
  • Rotavirus
  • Hepatitis A
  • Flu

3-21 Year Olds:

Well child visits aren’t just for babies. All young people should receive a well visit every year to keep them healthy. You can get a reward if your child between the ages of 3-21 has a well visit at least once this year.

Preventative Health Visit

Adults 20 and over should see their PCP once a year for a preventive health visit. This visit may cover the ABC’s of health:

  • A1c Check- checking your blood sugar levels
  • Blood Pressure check
  • Cholesterol or Cancer screenings

Immunizations for Teens

Kids turning 13 should have the following vaccines before their 13th birthday: one dose of meningococcal vaccine, one Tdap vaccine and the complete human papillomavirus (HPV) vaccine series (2 doses).

Lead Screening

Children under 2 should have 2 lead screenings by their 2nd birthday – 1 at 12 months, and a second by 24 months.

Children 24 months to 72 months should receive at least 1 test before the age of 6 if no tests were performed by their 2nd birthday.

Diabetes A1C (Blood Sugar)

Adults age 18-75 with diabetes (type 1 or type 2): Call or go to your doctor to get A1C blood test screening lab slip.  Next, go to a lab in the Health Plan Network to have get the Diabetes A1C Test

Prenatal

Visit doctor within the first three months (12 weeks) of becoming pregnant or within 42 days of becoming a Health Plan member.

Prenatal Immunization Status

Pregnant women should receive influenza and Tdap vaccinations during their pregnancy to protect mom and baby.

Postpartum Visit

Women who gave birth should have at least one postpartum visit on or between 7 and 84 days after delivery.

Postpartum Depression Screening

All postpartum women should receive a depression screening after delivery, and follow up care if needed.

Breast Cancer Screening

Women 50-74 years old: Call or go to your doctor to ask for a mammogram. Get your mammogram done and prevent or catch breast cancer early.

Cervical Cancer Screening

Make an appointment with your doctor or an OB/GYN to get tested for cervical cancer.  The doctor will do a pap smear.

Colorectal Cancer Screening

Adults between the ages of 50–75 should get screened for colorectal cancer with any of the following tests:

  • Annual fecal occult blood test
  • Flexible sigmoidoscopy every 5 years
  • Colonoscopy every 10 years
  • Computed tomography colonography every 5 years, stool DNA test every 3 years

Flu Shot

Anyone 6 months or older: Make an appointment with your doctor or visit a local pharmacy or clinic to get your annual flu shot from 1/1/23-12/31/23.

Please note: Gift cards cannot be used to purchase products that would pose health risks such as alcohol, tobacco and firearms.
