Choose Health Plan of San Joaquin/Mountain Valley Health Plan (“Health Plan”)

If you live in Alpine, El Dorado, San Joaquin or Stanislaus counties, and are eligible for Medi-Cal, you can become a Health Plan member! At Health Plan, you will have access to high quality health care in a timely, caring and culturally sensitive manner.

About Health Plan

Health Plan has been serving members enrolled in publicly-funded programs in the heart of the Central Valley since 1996. Health Plan is part of the two-plan model and serves as the local initiative in Alpine, El Dorado, San Joaquin and Stanislaus counties. We have offices in French Camp, Modesto, and Placerville.

We continuously work to improve the health of our community by providing access to high quality cost-effective health care to over 400,000 members – serving families in Alpine, El Dorado, San Joaquin and Stanislaus counties.

We can help you get to your medical appointments and health education classes. Once you have your appointment scheduled, call to talk with our friendly Customer Service staff to talk about transportation options.

  We speak your language:
Our staff speak many languages and you have access to interpreter services by phone. We can also arrange to have an interpreter meet you at doctor’s appointments.

  Peace of mind when your doctor can’t be reached:
Talk with a nurse or doctor from HealthReach, our health advice line, 24 hours a day.
