The ECM Birth Equity POF is for pregnant and postpartum individuals who are subject to racial and ethnic disparities as defined by California public health data on maternal morbidity and mortality.
Health Plan would like to inform you that pregnant and postpartum members who meet the following ethnic groups are eligible for ECM Birth Equity POF (for a 12-month period):
- African American
- American Indian
- Alaska Native or
- Pacific Islander
Reminder:
- Referrals can come from any source:
- Member/Self Refer
- Family Referral
- Primary Care Physician (PCP) Referral
- Other ECM-CS Referrals
Qualifying members can be referred to ECM by following the steps below:
