(French Camp, CA) – Health Plan launched a Community Reinvestment Program in March 2024 with an investment of $100M over the next three years. Health Plan aims to transform care delivery and expand provider access to improve the quality of health care in the communities we serve; Alpine, El Dorado, San Joaquin and Stanislaus Counties.

Health Plan has posted two additional grant initiatives of the Community Reinvestment Program:

Capital Projects Grant
The Capital Projects Grant is open to community stakeholders and providers who seek funding to purchase, construct or implement capitol assets such as building and equipment to support planning and implementation of projects that have the goal of improving facilities or promoting high-quality, equitable, and accessible care.
Grant amount: $2.5 million max per proposal

General Grant
Health Plan’s general grants are for a broader range of initiatives that meet community reinvestment objectives. They are open to health care, community based, and other community benefit organizations that seek support to improve care and services for projects and programs aligned with four of Health Plan’s reinvestment areas of focus.
Grant amount: Awards based on proposal review and qualifications
