Healthcare Effectiveness Data and Information Set (HEDIS) Medical Record Review (MRR) season is approaching.
Medical Record Requests will be faxed to selected providers by February 2025.
IMPORTANT: Please note Health Plan is willing to assist with medical record procurement. Therefore, Health Plan does not pay medical record copy fees. Please send an email to HEDIS@hpsj.com if you will require assistance with record collection.
RECORDS COLLECTION DEADLNE: March 28, 2025. Records received after this date may not be processed.

Note: This process is TIME SENSITIVE in preparation Health Plan recommends the following:
- Designate at least two contacts for the collection and/or coordination of records.
- If you have an Electronic Medical Record (EMR) system that allows for remote access, please contact Health Plan’s HEDIS team at HEDIS@hpsj.com to coordinate set-up. Direct EMR capture is a no-cost service for Health Plan contracted providers.
