NEW from Health Plan of San Joaquin (HPSJ) – Handy, One-Page Vaccination Chart for Children Well-Child Check-Up & Shots: Children from birth to 17 years old
(French Camp, CA) – Health Plan of San Joaquin (HPSJ) has a new one-page way to guide parents in keeping up with their child’s Well-Child check-ups, vaccinations, when to screen for hearing and vision, blood screening, and when to ask about flu shots. The chart, in English and Spanish, is based on recommendations from the Centers for Disease Control and Prevention (CDC).
“Like all busy parents in our community, HPSJ staff know how tough it can be to keep up with the Well-Child Visits and shots vital to keep our children healthy,” said HPSJ staffer Jill Center.
This fact sheet has been created – with parent help – to be a clear, all-in-one place where Health Plan of San Joaquin members (and every local parent) can map the route to their children’s best health. It’s even been crafted as a fun tool for kids to start helping out with their own good health habits as they grow. “This is the single sheet all our parents – and providers and community partners – have been waiting for!” said HPSJ’s Center.
