Why?
Since July 1, 2016, new regulatory standards have been in place for plans to capture and maintain accurate provider data from all of their network providers. Our goal is to finish this required task through our provider/HPSJ partnerships – and to do this as efficiently as possible, as we know how busy your facility practice is.
To help your practice submit the required documents, HPSJ has made the fillable forms available on our website and provider portal (DRE) for you to complete and return via:
1. Fax – One of the easiest ways to provide this information; just complete the fillable form and return by December 30, 2016, to this secure HPSJ fax: 209-461-2565.
2. Email – Send an email with the form attached to providerservices@hspj.com.
