Health Plan conducts an annual Provider Satisfaction Survey for its contracted providers to capture their rate of satisfaction with Health Plan. Information obtained from this survey allows Health Plan to measure how well we are meeting our providers’ expectations and needs, and also to identify areas of improvement.
A third-party vendor for Health Plan has mailed out surveys on November 4, 2024, requesting completion within 30 days. Data collection will end in January 2024. In order to better serve our providers, we request your feedback with this survey. The survey takes no more than 10 minutes to complete. The results will assist us in identifying our strengths and opportunities.
There are two ways to respond:
- By mail – address is on the survey
- Online – instructions will be included on the mailed survey
To address your concerns and contribute to the success of your practice, your confidential feedback is vital.
- We want to know how we are doing.
- We want to know what practical things we can improve.
- We need your feedback to identify how we can be a better partner and support you.
Your responses are confidential, and data will be aggregated before being provided to the Health Plan for review and improvement opportunities.
