Health Plan is issuing revised 1099-Misc forms due to necessary corrections (incorrect provider name) in some of the forms. The updated forms will be mailed out by 02/18/2025.
If you receive a revised 1099-Misc, please use it for your records and tax filing purposes. If you have already received an initial form with an incorrect provider, kindly disregard it and refer to the revised version instead.
