The following is a update regarding the Well Child Visits in the First 30 Months of Life
(W30).

Description

Visit must occur with a PCP, but the PCP does not have to be the practitioner assigned to the child.

Reported by two Rates:

  1. Well visits in the first 15 Months during the measurement year (6 or More well visits) on or before the 15-month birthday.
  2. Well visits for age 15 Months thru 30 Months during the measurement year (2 or more well child visits).

Please note that a total of 8 or more well-visits at/by 30 months of life.
